Because the SQP Store is customized based on your hospital’s program participation, only primary contacts and marketing contacts have access. These roles will need to log in using the information provided via email.
Add at least one Surgical Quality Partner plaque to your cart in the SQP Store. At checkout, you will be able to enter your redemption code (provided via email to your primary contact and marketing contact), which will reduce your total by the price of one plaque. Additional plaques can be ordered at the same time as your complimentary plaque. Please note that only one redemption code can be applied per order. If you are ordering on behalf of multiple hospitals, you will need to submit separate orders for each code.
Redemption codes are limited to one use per hospital/facility location. If your code has already been redeemed, another contact from your institution has claimed the complimentary plaque. Anyone can place orders to purchase additional plaques at no limit.
Once logged in to the SQP Store, navigate through the Program Materials menu to find the digital marketing kits for each program that your institution participates in. Click the “Download” button on each marketing kit to download the files to your device. We recommend using a computer/laptop as file sizes are large and download times may vary.

Access to program-specific marketing toolkits is customized to your institution based on our program participation records at each hospital/facility location. For certain programs (NSQIP, NSQIP Pediatric, and TQIP), Surgical Quality Partner materials are only available after 1 year of participation. If you are a primary/marketing contact for multiple hospitals, you will see digital marketing toolkits for all programs that any of your hospitals participate in. Please contact us at SQP@facs.org if you are unsure which programs apply to which hospitals.
