Quality Programs

Business Associate and Data Use Agreement with the ACS

You can view and electronically sign the Business Associate and Data Use Agreement with the American College of Surgeons (ACS). The ACS has developed a standard Business Associate and Data Use Agreement for all of the quality improvement programs administered by the College. The new agreement includes the new HIPAA security rule requirements.

You only need to sign one Business Associate and Data Use agreement to cover all the quality improvement programs your institution participates in.

Instructions

(Please read first.)

Please follow these steps to review and electronically sign the agreement. This procedure should be followed by the authorized signer at your institution:

  1. Read these instructions before you select "Continue" below.
  2. Fill in your name and e-mail address.
  3. You will receive an e-mail with an access code.
  4. Return to the website and input the access code.
  5. Once you have submitted the access code, the agreement will appear and it includes fillable fields to enter the name of the covered entity, FEIN/Tax ID, address, and contact information through the Notices section on page 9.
  6. You must confirm acceptance of the document on this screen. (You can print the document, if desired.)
  7. After acceptance of the document, you will receive a confirmation e-mail and the ACS will be notified of document acceptance. The e-mail will contain instructions on how to print the document, if desired.
  8. The ACS will then electronically countersign the document.
  9. You will then receive another notification e-mail that the College has countersigned the document. The e-mail will contain instructions on how to print the document, if desired.
  10. Once you have reviewed the instructions, click on "Continue" below.
