Initial Applicants

To apply for any level of GSV participation, including the new GSV (Age Friendly) Level, register through the Initial Application link below to begin the application process. New ACS Geriatric Surgery Verification Program participants will receive a login to the ACS Quality Portal (QPort) once a submitted application has been processed.
