Applications for Associate Fellow Membership are accepted year-round.

How to Become an Associate Fellow

Step 1: Confirm Your Eligibility 

  • Graduated from an accredited medical school (US, Canada, or international)
  • Completed residency training
    • Please note that if you are in a fellowship program, your membership is still considered in the Resident category.
  • Hold a current, unrestricted medical license
  • Started surgical practice and have a hospital or academic appointment

Note: Associate Fellow membership is for surgeons within 6 years of first independent practice.

Step 2: Gather What You’ll Need

Have the following ready before starting your application. All documents must be in English.

Personal and Professional Information

  • Medical school and residency details
  • Additional surgical training

Licensing and Certification

  • Medical license number, state, and issue/expiration dates
  • Certification board name, certificate number, and initial certification date

Step 3: Create or Log Into Your ACS Account

To apply, you’ll need an ACS account.

Step 4: Submit Your Application

Once submitted:

  • You'll become an Associate Fellow (you may not use “FACS" yet)

Step 5: Fees & Dues (US and Canada)

  • Application Fee: $75 (Waived if you're a current Resident Member)
  • Annual Dues: $300 (Due July 1 each year)

Fees vary internationally based on World Bank income categories:

  • Upper‑middle/high‑income: Fee $75 | Dues $150
  • Lower‑middle‑income: Fee $50 | Dues $75
  • Low‑income: Fee $25 | Dues $38

Looking Ahead—to Fellowship (FACS)

Being an Associate Fellow puts you on the path toward full Fellowship (FACS), which requires:

  • One (1) year of independent practice in the US and Canada or 3 years of independent practice for surgeons practicing internationally
  • Board certification by an appropriate surgical specialty board
  • Submission of surgical case list, references, interview, and eventual induction at Clinical Congress

As your application for Fellowship progresses, you'll maintain Associate Fellow membership and enjoy continuous access to benefits until induction as Fellow (FACS).
