ACS NSQIP QVP Small and Rural Small: Sites which perform fewer than 1,680 cases/year Rural: Sites whose zip codes fall under Rural Urban Commuter Area (RUCA) data codes 7.0 through 10.6 U.S. and international sites	Focused: Annual Fee of $10,000† Comprehensive: Annual fee of $14,000†	New sites will be invoiced upon execution of the hospital participation agreement. Renewing sites will be invoiced 60 days before the end of the current participation period. Site must have complete 30-day follow-up for a minimum of 80 percent of cases within a SAR period to receive a report. *One complimentary training session is allocated for the initial SCR for new site enrollment only. †Currency must be in U.S. Dollars
