CLP Appointment Process

The Commission on Cancer (CoC) Cancer Liaison Physician (CLP) is an integral and required member of a CoC-accredited cancer program’s cancer committee.

The CLP:

  • Is selected and appointed by the cancer committee.
  • Can be a physician of any specialty who is an active member of the medical staff.
  • Fulfills the role of physician quality leader of the cancer committee.
  • Serves as the alternate for the cancer committee chair and oversees cancer committee meetings if the chair is not in attendance.

It is permissible for the CLP to also serve as the cancer committee chair, but it is encouraged that the CLP role and the chair role be filled by two individuals.

Before selecting a CLP, it is important to review information about the duties and expectations of the role and ensure that the individual selected is willing and able to fulfill the role as defined.

Steps to Appointing a CLP

After appointing your CLP and documenting the appointment in the cancer committee minutes, your site’s Primary Contact must add the CLP to your Site Contacts within the CoC Quality Portal (QPort). If you need to remove an existing CLP, please inform your Primary Contact. Only the Primary Contact can add/edit/delete contacts. Once the request for the new CLP has been processed, an appointment email will be sent to the CLP with their QPort login information. Please allow 3-5 business days for processing new contacts.

Appointing a CLP

  1. The Primary Contact logs into QPort.
(use same username and password as CoC Datalinks)
  2. Click on the Site Contacts page.
  3. At bottom of page, click on 'Add New Contact'.
(Only the Primary Contact can add/edit/delete contacts)
  4. Enter CLP's role and contact information.
  5. Click the Save button.

The new information will be sent directly to the Cancer Liaison Program for processing. An appointment email will be sent to the CLP with their CoC QPort login information.

Removing a Former CLP

  1. Log into QPort
(use same username and password as CoC Datalinks)
  2. Click on the Site Contacts page.
(Only the Primary Contact can add/edit/delete contacts)
  3. Click on the former CLP’s name to open contact page.
  4. At bottom of page, click on ‘Remove Contact’

Confirmation

If you have any difficulty with this process, please e-mail coc@facs.org.

Please note: If your accredited facility does not have a CLP in place, you are in jeopardy of being in noncompliance with CoC accreditation standards. It is important that an appointment is made as soon as possible and that an individual is designated as the CLP in QPort. If a CLP is not appointed for your facility and recorded in QPort, the CoC recommends that the Cancer Committee Chair be listed as the interim CLP until an official appointment is made.
