Participation Fee for International Data Collection Centers	Annual fee of $10,350	Center will be invoiced upon submission of Data Collection Center Application; fee must be remitted prior to receiving access to MBSCR training or database. Conversion Application Fee (converting between Accredited designation levels)**	One-time fee of $1,000 only applicable for Accredited centers applying to change designation level outside of existing renewal cycle. Centers applying to convert at the time of scheduled renewal will not be invoiced this fee.	Center will be invoiced upon request for off-cycle Conversion Application. Additional Site Survey Fee^†**	One-time fee of $3,000 only applicable when: MBSAQIP Committee determines that an additional off-cycle survey is required OR A participating center seeks to convert status to an Accreditation designation off-cycle. Centers applying to convert at the time of scheduled renewal will not be invoiced this fee.	Center will be invoiced upon determination of a required off-cycle/conversion survey. Data Integrity Audit Fee	One-time fee of $3,500	Center will be invoiced upon notification for Data Integrity Audit. Please contact clinicalsupport@mbsaqip.org regarding the Data Integrity Audit process.	Pre-audit meeting with Clinical Support Team Private and secure remote data audit of random sample cases Center will receive a detailed summary report of the audit results

*Center must have complete 30-day follow-up for a minimum of 80% of cases within a SAR period to receive a report.

**Designation as an MBSAQIP Accredited center is only available at this time to centers within the United States and Canada.

^†This service is only provided for centers in the United States and Canada that are seeking accreditation.
