Fillable PDF Forms for CoC Standards 5.3-5.6 Are No Longer Compliant Starting in 2026

Use of the fillable PDF forms for the synoptic requirements for CoC Standards 5.3-5.6 will no longer be compliant starting January 1, 2026.

The fillable PDF option for achieving compliance with the synoptic requirements of the CoC Operative Standards (Standards 5.3-5.6) was offered as a stop-gap measure for programs unable to incorporate the synoptic elements/responses into their operative reports of record by the January 2023 deadline. However, by January 1, 2026, programs must comply with the requirements of the standards for applicable cases and incorporate the required synoptic elements/responses into the operative report of record.

Cases occurring January 1, 2026, or later that use the fillable PDF forms will be considered non-compliant with the pertinent standard. Please email cssp@facs.org with questions.
