Surgeons must make hundreds of decisions correctly each day to provide quality patient care and outcomes for their patients. ACS Cognitive Simulations: Cases Essential to Surgical Practice (CogSim) is an e-learning product using cognitive simulation to teach PGY-3+ surgical residents how to approach complex decision making and management of surgical problems, from preoperative assessment to perioperative care and management, including complications. Sharpening these skills through interactive online cases can enhance residents’ ability to care for patients through reinforcing best practices and improving resident self-efficacy.
Cognitive simulation allows residents to practice complex tasks or decision-making scenarios in a safe and controlled environment. They are designed to assess, educate and help learners self-identify gaps in their understanding of material or application of knowledge
The product consists of 65 cases scenarios in topics essential to general surgical practice in the following areas:
The ACS Division of Education defined the essential knowledge areas, conditions and key learning points to be included in CogSim in conjunction with a steering committee of surgical educators. Cases are authored by expert surgical educators and peer reviewed by two additional surgeons. The development of cases is ongoing.

ACS CogSim is available to senior residents, from PGY-3 onwards. Programs can elect to enroll:
Enrollment provides a subscription for each resident and a complimentary administrator account. Enrollment for 2025–2026 costs $155/resident. Residents have access to 65 case scenarios until June 30, 2026.
CogSim is purchased by residency programs for their PGY3/4/5 residents.
