Requirements

The levels of verification are a progressive series in that to achieve a higher level all other levels below it must be satisfied. Starting with the lowest level (1) and moving to the highest level (5), the verification levels are:

Level II	Verification of Course Objectives	Response to case presentation questions and/or report card; and course evaluation form
Level III	Verification of Knowledge & Skills	Pre-/Post-test and report card; course evaluation form
Level IV	Verification of Preceptorial Experience	Mentored skills training; PI CME
Level V	Verification of Satisfactory Patient Outcomes	Case log results; post-op survey and chart audit
