This report reviews all of the options for surgical and radiological management of variceal hemorrhage. The authors stress the importance of retaining the skills to perform open surgical shunts (such as the small-caliber portacaval H-graft) in settings where TIPS may not be available or for patients with life-threatening, ongoing, bleeding.
