Step 5: Four Months after the Meeting

All templates referenced on this page will be included in your approval e-mail that you will receive once your application has been approved. These documents will be customized for your meeting. Please use only the templates included in your approval e-mail when completing the rest of the CME process.

Please submit the following:

Final Report Form: Must be sent to the ACS within 4 months after the event.

Compile comments from the on-site CME Program Evaluation Forms completed at the meeting and enter into questions 8–11 on the Final Report Form.

All comments from the completed Post-Activity Surveys must be included in the Post-Activity Survey Summary of the Final Report Form. This should be submitted as an attached document.

Another copy of the Program Book distributed at the event.

Three samples of completed Self-Assessment tests, if the activity included a self-assessment component.

Please note: Once your final report has been processed by Chapter Services and the Division of Education, you will receive e-mail notification when the file has been closed. The ACS will not process CME applications without having closed previous event files. There are no exceptions to this. Chapters that are requesting CME credits for several different events during the course of the year should consider submitting applications early, since the ACS will not process CME applications without having closed previous CME files.
