Step 2: One Week (5 Business Days) Prior to the Meeting

All templates referenced on this page will be included in your approval e-mail that you will receive once your application has been approved. These documents will be customized for your meeting. Please use only the templates included in your approval e-mail when completing the rest of the CME process.

Following the ACS CME approval, please submit, at one time, the following items:

Faculty/Speaker Disclosure Form
Please use this Faculty Disclosure Form. This form is to be used for both speakers and moderators. 

If an individual discloses a conflict of interest, the program chair or a moderator (designated MD/DO official) must complete the bottom portion of the Disclosure Form pertaining to the management of the conflict. The designated official should select a method of managing the conflict and sign the Form after discussing the conflict with the individual.

Once all disclosure forms have been received and reviewed, the Disclosure Summary Excel template should be completed and returned to ACS at least 5 business days prior to the start of the activity (rather than submitting each Disclosure Form).

On-Site Disclosure List
The On-Site Disclosure List may be:

  1. Printed directly into the meeting program book
  2. Inserted into the program at the event, providing that the following text is printed into the program book:
    “In compliance with ACCME Accreditation Criteria, the American College of Surgeons, as the accredited provider of this activity, must ensure that anyone in a position to control the content of the educational activity has disclosed all relevant financial relationships with any commercial interest. All reported conflicts are managed by a designated official to ensure a bias-free presentation. Please see the insert to this program for the complete disclosure list.” 

Letters of Agreement for Grants
If grants are supporting the chapter event, completed Letters of Agreement (LOAs) for each grant must be submitted to ACS.
