Description: Session will describe shift away from fee-for-service(FFS)and domains it will impact(appropriateness related to indication,age and risk group,greater use of gadgets to compete for market share,greater use of output setting surgery).Describe what we thinkVs.know about the implications of shifting away from FSS-based on quasi-experimental data(surgical care in USvsEurope, before/after data,regional comparisons).Describe concepts of value-based purchasing(VBP)in surgery and show examples.Explain how ACO,warranties and bundled payments include aspects of VBP and how far away this state is from reality for surgeons.Describe financial threat of shift to VBP,implications for incentives,workload,work force if we take 1in3 dollars out of surgery and shift from revenue to cost center.How market forces are supporting this transition and how to lead the way from FFSto VBP-especially,how to do both at the same time,while transitioning.
