Step 6: Final Report

The template referenced on this page will be included in the Application approval email. Please submit the following to CPDA@facs.org four months after the educational activity, in order to close the CME file.

Final Report

The following supporting documentation is required as part of the submission:

  • Three samples of evaluations completed by learners after participating in the educational activity
  • A summary of all comments from questions 10-15 on the evaluations completed by learners after the educational activity. This should be submitted as an attached document (Word, Excel, export from SurveyMonkey or other evaluation tool)
  • Three samples of Post Activity Surveys completed by learners two months after the educational activity.
  • A summary of all comments on the Post-Activity Surveys provided by learners two months after the educational activity. This should be submitted as an attached document (Word, Excel, export from SurveyMonkey or other evaluation tool)
  • Please note: Once your Final Report has been reviewed by the CPDA Team, you will receive an email notification once the required information is complete and the CME file has been closed. The ACS will not accredit future educational activities until past CME files have been fully closed.
