Step 3: Additional Required Documentation

The templates referenced on this page will be included in the Application Approval e-mail, and will be customized for your educational activity. Each component is due to CPDA@facs.org for review and approval at least five business days prior to the start of your educational activity. 

Following the ACS CME Credit approval, please submit, at one time, the following items:

Faculty/Speaker Disclosure Information

  • This form is to be used for speakers, moderators, discussants, abstract presenters, etc. If an individual discloses a conflict of interest, the program chair or a moderator (a designated MD/DO official) must complete the bottom portion of the Disclosure Form pertaining to the management of the conflict. The designated official should select a method of managing the conflict and sign the Form after discussing the conflict with the individual.
  • Once all disclosure forms have been received and reviewed, the Disclosure Summary Excel template should be completed and returned to ACS at least 5 business days prior to the start of the activity (rather than submitting each Disclosure Form).

Disclosure List

The Disclosure List may be either:

  • Printed directly into the program materials
    -or-
  • Provided as an insert to the program (for live meetings), if the following text is printed in the program:
    “In compliance with ACCME Accreditation Criteria, the American College of Surgeons, as the accredited provider of this activity, must ensure that anyone in a position to control the content of the educational activity has disclosed all relevant financial relationships with any commercial interest. All reported conflicts are managed by a designated official to ensure a bias-free presentation. Please see the insert to this program for the complete disclosure list.”

Letters of Agreement for Commercial Support (“Educational Grants”)

If the educational activity is receiving commercial support (either monetary or in-kind educational grants), a letter of agreement (LOA) must be signed by the commercial company, Joint Providership Partner, and ACS prior to the start of the educational activity.

A Draft of the Program Materials (prior to finalizing)

Upon approval, Program Requirements that contain required verbiage for your educational activity will be provided. These include the ACCME Accreditation Statement, AMA Credit Statement, learning objectives, disclosure information, and commercial support/promotion acknowledgements.
