Step 2: CME Application

The CME Credit Application is due to CPDA@facs.org at least 3 months prior to the start of the educational activity. Please note that Applications received after the deadline will incur a late fee according to the fee structure.

The CME Credit Application will be provided upon review of the CME Activity Form (previous step).

The following documents must be included with the CME Credit Application:

  • All Planning Committee Disclosure Forms
  • Planning Minutes/Narrative, including the following components:
    • How programming was developed for the event, any theme or CME topics selected, speakers identified
    • List of the planning committee members
    • Date(s) of planning meeting/conference call
    • A statement by the Committee Chair stating that “Any member who has indicated a potential conflict of interest will be required to recuse themselves from any discussion where their stated conflict could influence the content of the program.”
  • Post-Test (if applying for Self-Assessment Credit; optional only for live meetings)
  • Exhibitor Prospectus or mailings sent to commercial companies (if soliciting commercial support or commercial promotion)

Upon review and approval of your CME Credit Application, you will receive an email notifying you of your approved credit information, next steps of the CME process, and additional documentation required prior to the start of your educational activity.
