Verification Site Visit Outcomes

I.Verified" Certificate for three years

– No deficiencies identified 

II. "Verified" – Certificate for one year

  1. No “Type I” deficiencies identified              
  2. Three or less “Type II” deficiencies identified
  3. The “Type II” deficiencies must be corrected within 12 months from the date of the notification in order to extend certification to 3 years.  At the discretion of the VRC, an on-site Focus Visit may be required.
  1. If the deficiencies are corrected, the certificate will be extended to 3 years (from the date of the first site visit)
  2. If the deficiencies are not corrected within 12 months, the hospital will no longer be verified and a “full site visit” process will be required to achieve verification. 

III. "Not Verified" – Deficiencies identified

  1. A “Type I” deficiency identified
  2. More than three “Type II” deficiencies identified
  3. Focus Review – A successful Focus Review is required to achieve verification, which may be by mail or an on-site Focus Visit (6 to 12 months from date of letter), at the discretion of the VRC.

  1. If successful, the certificate of verification will be for three years from the date of the first site visit.      
  2. If not successful, a “full site visit” process will be required.

Type I Deficiency – Requirement that must be in place at time of site visit in order to achieve verification.

Type II Deficiency – Less substantive requirement, that if three or less in number, will allow a trauma program to be verified for one year.  These deficiencies must be resolved within that year for the hospital to maintain verification as a trauma center.
