How to Order SESAP 13

You may order SESAP online through this Web site through a secured server; or you may place your order by phone, or by mail. You must use a credit card (Visa, Mastercard, or American Express) if you order online or by phone.

To order by phone, call the SESAP order section at 800-251-3775 or 312-202-5474. When ordering by phone be prepared to give your cardholder name, credit card account number and expiration date, and a mailing address.

The enrollment fees for SESAP are:

• Fellows—$395
• Associate Fellows—$310
• Resident Members—$190
• Affiliate MD/DO Members—$395
• Nonmember Residents in an accredited program—$255
  (NOTE: Residents cannot order online; a letter must be enclosed, and the order can be placed only by fax or mail.)
• Others—$560
