Application Information

COURSE REGISTRATION: Please complete the application form and submit it to Ms. Laura Meyer, administrative assistant, Division of Education, by faxed transmission or mail, as detailed on the application form. Applications must be postmarked no later than Friday, May 23, 2008. Faxes and envelopes with postmarks after this date will not be considered.

ENROLLMENT: The course is limited to 30 participants. Letters of notification regarding course acceptance will be mailed by Friday, May 30, 2008. All participants are expected to attend the entire course and remain until the conclusion of the course.

COURSE FEE: Please do not include payment when submitting the application form. Full payment of the course fee of $3,350 for ACS members and $3,850 for nonmembers will be due upon written acceptance into the course. The fee covers tuition, all course materials, six nights lodging, and all meals, break refreshments, and special events. Course participants must submit full payment by Thursday, July 3, 2008, to reserve their place in the course. Additional details will be enclosed with the notification letters.

CANCELLATIONS AND REFUNDS: Participants who find it necessary to cancel their registrations will receive a full refund, less an administrative fee of $50, provided the cancellation is received by close of business on Friday, July 25, 2008. Registrants who cancel after Friday, July 25, 2008, will not receive a refund unless an alternate from the course=s official waiting list can be confirmed.

ACCREDITATION: The American College of Surgeons is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

CME CREDIT: The American College of Surgeons designates this educational activity for a maximum of 46.5 AMA PRA Category 1 Credits. Physicians should only claim credit commensurate with the extent of their participation in the activity.
