Informs programs who their assigned surveyor is and includes the survey fee for the upcoming survey. |
SAR Access for (specified year) Survey |

Announces that the SAR has been updated with the new year and is ready for completion in preparation for survey or for program activity tracking between survey years. |
Survey Confirmation |

Provides confirmed survey date between surveyor and program for programs due for survey. |
Post-Survey Evaluation Form Completion Requested |
1-Day Post Survey |
Request to complete the Post-Survey Evaluation following survey for feedback purposes. |
Initial Performance Report |

Sent when the survey review is complete and results are available. |
Updated Performance Report |

Sent whenever a Performance Report has been updated with corrections, appeals, and/or deficiency resolutions. |
SAR Archival and Availability of New SAR |
