Survey Extension Request Policy
Due to extenuating circumstances, a survey extension request may be requested. The Cancer Committee Chair or the Administrator of the facility must submit a request for extension in writing to Lisa Landvogt, American College of Surgeons, Commission on Cancer 633 North Saint Clair Street, Chicago, IL 60611 within 45 days of initial survey notification. The request letter must include details of the rationale for the extension request, proposed plan and timeline to resolve the issue necessitating the extension request. Facilities will be notified of the extension request decision within 14 days of receiving the written request.
