American College of Surgeons: Cancer Programs

2011 Survey Confirmation

Your cancer program, {Facility Name & FIN}, has been confirmed for survey by the American College of Surgeons Commission on Cancer (CoC) at {Time} on {Day of Week & Date (mm/dd/2011)} by {surveyor Name(s)}.

The assigned surveyor(s) profile is available on the Survey-Specific Resources page of the American College of Surgeons Web site at http://www.facs.org/cancer/coc/surveyorprofiles.html. The surveyor's contact information is available through the 'Surveyor Contact List' link located on the CoC Datalinks Activity Menu.

The survey fee for 2011 is $6,500 for individual cancer programs or $6,500 plus $5,000 for each hospital within a network. If your program requests to change the survey date or cancel the survey, a $1,000 change/cancellation fee is required. Invoices are electronically sent to the registrar at least one month before the survey takes place. Payment of the fee is due within 30 days of confirmed survey date.

Documentation that is to be provided to the surveyor a minimum of 2 weeks prior to the on-site visit is listed below. Documentation that is to be provided by NCI-designated Comprehensive Cancer Center programs is listed separately on page 9 of the Cancer Program Standards 2009, Revised Edition. Pre-survey documentation is provided by uploading to the SAR through the link, 'SAR Documentation', located on the SAR Menu. ALL survey documentation is to be uploaded no later than 14 calendar days before the scheduled survey. No paper documents will be accepted.

A list of additional documentation that is to be provided to the surveyor(s) during the on-site visit can be found in the Cancer Program Standards 2009, Revised Edition on page nine.

During the on-site visit the surveyor(s) will review 30 pathology reports and the corresponding cancer registry abstract (per facility) in order to evaluate the inclusion of the scientifically validated data items in the pathology reports and the timeliness of abstracted data.
