04CA-0007 Cancer Program Standards 2009 Revised Edition
This publication includes all revisions made to Cancer Program Standards 2004 by the Accreditation Committee of the CoC through 2009, and includes modified interpretations of those standards that apply specifically to pediatric hospitals, pediatric components of larger facilities, Veterans Affairs (VA) facilities, and NCI-designated comprehensive cancer centers. The activities of the Commission on Cancer (CoC) Accreditation Program have been designed to ensure that the structure and processes necessary for quality cancer care in a facility are in place. This publication sets forth the 36 cancer program standards required for CoC cancer program accreditation. The manual replaces all previous cancer program standards-related publications produced by the CoC, including Cancer Program Standards 2004.

Facility Oncology Registry Data Standards (FORDS) for 2010
Registry data management is an essential element of Commission on Cancer (CoC)-accredited cancer programs. The manual, Facility Oncology Registry Data Standards (FORDS): Revised for 2010, provides definitions and detailed instructions for coding patient diagnosis, treatment, and outcomes. It also describes the types of cases that must be abstracted and followed and explains the relationships among data items. Cancer registry data maintained with standardized quality control mechanisms supports meaningful evaluation and analysis. FORDS: Revised for 2010 contains all the additional changes made since the initial release of the manual in 2002. Implementation of the FORDS: Revised for 2010 manual, is required of all CoC-accredited cancer programs.

Staging Desk Reference
The American Joint Committee on Cancer (AJCC) has developed the Staging Desk Reference that can serve as a handy tool when abstracting cancer cases. The Staging Desk Reference is not intended to replace the Seventh Edition AJCC Cancer Staging Manual, but rather serve as a useful resource that can quickly and easily provide specific information relevant to staging.
The first section, General Staging Rules, provides the TNM rules formatted for quick access. Chapter 1 of the Cancer Staging Manual provides more information and context, but the basic rules are provided in the Staging Desk Reference. The Cross Reference section, as the name implies, was created to help registrars quickly match ICD-0 codes, CSv2 schemas, and AJCC Chapters. Two frequently used cross references are provided in this section, the first organized by ICD-O-3 Site Code, and the second by AJCC Chapter. These features provide users with two options to find information they need. This booklet will be particularly useful when trying to determine which AJCC staging form to use for a particular code. Finally, the Head and Neck Nodes section provides a helpful tool for identifying nodes in an often confusing part of the anatomy.
