FORDS: Revised for 2009

This edition incorporates the changes in staging requirements for 2008 that were posted as separate addenda at that time, expanded information for coding the various types of embolization used with cancer patients, and a new rule stating that in utero diagnosis and treatment dates can now be coded to the exact date rather than to the date of birth.

For most purposes, this version of FORDS should replace all earlier editions. That is because item descriptions indicate when new items should replace obsolete ones, and this update makes clarifications where questions arose in the past.

We strongly encourage you to read the Preface and review Appendix C for an explanation of the changes made to this revision of FORDS.
