FORDS: Revised for 2011
This version of FORDS replaces all previous versions. Modifications include discontinuation of eight items, addition of seven CS site-specific factor requirements, a new code option for primary site breast surgery, ICD-10-CM ranges for coding comorbidities and complications, and a number of clarifications. Registries are encouraged to read the Preface and review Appendix C for an overview of the changes.
All items in FORDS are required for CoC accredited cancer programs. Supplementary references are necessary for coding AJCC, collaborative stage, multiple primaries and histologies, hematopoietic tumors, and systemic agents.
